I am right now looking at the menu of the 80 individual pages at TOS. I won't post the link as I believe there are still some issues regarding that site and this upon which I care not to encroach.
The format is very inconvenient, but the manual seems to be all there.
Unfortunately, according to the listing of contents of the most recent MoHPC DVD set, the manual you seek doesn't seem to be there.

I have the book. I thought I had already scanned it and sent it to Dave, but I guess I didn't. I'll scan it and send to Dave so he can put it on the next DVD. And no, I won't send copies of the scan to others since I don't have the rights to do that.
